Major Scientific Achievements:

• Highlights in the immunophenotypic characterization of the normal, reactive and neoplastic T- and NK-cells, and discovery of new T- and NK-cell lymphoproliferative disorders

The Laboratory of Cytometry of the Department of Hematology of the CHP has been dedicated for a long time to the phenotypic characterization of the normal and reactive T-cells and NK-cells, and to the identification of the aberrant immunophenotypic patterns associated to different types of lymphoproliferative disorders (LPD) originating from T-cells and NK-cells. In 2003 our group described, for the first time, a series of patients with clonal proliferations of CD4+ Large Granular Lymphocytes (LGL) who have clinical manifestations distinct from those usually observed in CD8+ Large Granular Lymphocyte Leukemia (LGLL). CD4+ LGLL is now recognized as a variant of T cell LGLL in the WHO classification of tumors of hematopoietic and lymphoid tissues.One year later, in 2004, we also described, for the first time, a series of patients with clonal proliferations of NK cells absent or low CD56 expression. Lymphoproliferative disorders of CD56-/+low NK cells are now known to occur in association with chronic viral infections, such as Hepatitis C virus or Human Immunodeficiency Viruses.Since 2009, we represent Portugal in the EuroFlow consortium, being co-responsible for the areas of T- and NK-cell neoplasms. Over the past few years, our group has published numerous papers on this subject.

• First outpatient clinics specialized in Mastocytosis and Cutaneous Lymphoma in Portugal

The Multidisciplinary Consultation for Cutaneous Lymphoma and Mastocytosis was created at the Centro Hospitalar do Porto (CHP) in 2003 and is certified to ISO 9001 since 2005. In 2004 it was recognized by the European Competence Network on Mastocytosis (ECNM) as an expert center for patients with mastocytosis, and by the Orphanet, as a center of expertise in cutaneous lymphomas and mastocytosis. Recently, it was accepted as a European Reference Center for patients with Cutaneous Lymphoma and Lymphocytosis in Portugal, integrated in the EuroBloodNet. Nowadays, nearly 400 adult patients with cutaneous lymphoma (mainly T-cell lymphomas) and 70 adult patients with mastocytosis are being followed up at this outpatient clinics, and around 50 children with cutaneous mastocytosis are being observed in pediatric dermatology out clinic, offering opportunities for clinical research in these rare hematological disorders. A working group on Mastocytosis was already formed at the CHP, which includes hematologists, dermatologists, pediatricians, orthopedists, immunologists, allergists, anesthetists and other medical specialties. A European Educational Meeting on Mastocytosis is being planned for October 2018. In addition, new projects aimed to better understand the clinical manifestations (cognitive and neuropsychiatric manifestations, bone disease, allergy, etc.) observed in patients with mastocytosis, are being planned, and we were invited to participate in the European Registry for patients with mastocytosis.

• New advances in the diagnosis and monitoring of bone marrow failure syndromes

The CHP has a significant number of patients with congenital and acquired bone marrow (BM) failure disorders, including Paroxysmal Nocturnal Hemoglobinuria (PNH), Fanconi Anemia (FA) and Myelodysplastic Syndromes (MDS). PNH is a rare disease caused by acquired mutations in the gene encoding for glycosylphosphatidylinositol (GPI) that lead to deficiency of the GPI-anchored proteins, and it may manifest as hemolytic anemia, thrombosis and/or BM failure. The Department of Hematology has a relatively large number of patients with PNH and is participating in the PNH Registry. The Laboratory of Cytometry has a large experience in PNH diagnosing and monitoring, as well as in characterizing the abnormal maturation patterns observed in the BM of patients with MDS. Concerning MDS, in the last years we developed a new flow cytometric tool for early detection of myelodysplasia in the peripheral blood (the MDS “thermometer”); the manuscript was already accepted for publication. Studies in patients with FA are being performed in collaboration with the Laboratory of Cytogenetic of ICBAS/UP, a reference Lab for the diagnosis of FA that participates in the International Fanconi Anemia Register (IFAR). A research project aimed for an early detection of myelodysplasia in patients with FA, previous to a malignant transformation, by combining cytogenetic and immunophenotypic studies, will start in 2018.

• First and single clinical laboratory in Europe performing endothelial cell studies by flow cytometry

In the last 5 years we have also explored the role of flow cytometry (FCM) in the immunophenotypic characterization of the endothelial cells (EC). Using FCM, in 2013 we quantified and characterized the circulating EC (CEC) and EC progenitors in patients with venous thromboembolism (VTE), as well as in patients with Myeloproliferative Neoplasms (MPN). In 2017, we correlated the serum levels of several EC molecules, the number of CEC and the clinical data from patients with VTE. Altogether, our results have suggested that CEC expressing activation related markers may reveal EC injury in patients with VTE and MPN, and that, together with soluble EC molecules, they may be used as biomarkers for thrombosis. Finally, in 2016 we developed a mechanic method for the isolation of the EC from large veins and we are now characterizing the EC from different origins, in normal and pathological conditions.

• Contributions to the diagnosis of congenital platelet defects

Congenital platelet defects (CPD) are responsible for a certain number of platelet disorders that manifest as thrombocytopenia and/or platelet dysfunction, thereby causing bleeding of variable severity. They may affect platelet surface glycoproteins (Gp) with important physiological roles, thus causing severe hemorrhagic diseases, such as Glanzmann Thrombasthenia (GpIIb/GpIIIa) and Bernard Soulier Syndrome (GpIb/GpV/GpIX). CPD involving the platelet granules, usually known as platelet storage pool diseases, have also been recognized. However, the precise Gp defects involved in other CPD with less severe clinical manifestations are largely unknown. In the last years we have combined studies classically used for platelet evaluation (platelet counts using hematological counters, platelet aggregation studies, platelet function assays, electron microscopy), flow cytometry based tests (platelet Gp quantification, platelet counts, etc.) and genetic studies to investigate the mechanisms involved in CPD. In collaboration with other UMIB groups (Clinical & Experimental Human Genomics), we are now characterizing the genetic defect in patients with CPD patients that are being followed up in our center (more than 100 patients). Some new genetic defects were already found, the results have already been presented in medical congresses, and a manuscript is being written.

• First and unique clinical laboratory in Europe dedicated to the study of Hodgkin Lymphoma by flow cytometry

For the last 3 years, our group has focused in the immunophenotypic study of Hodgkin and Reed Sternberg cells, in order to establish the criteria for the diagnosis of Hodgkin lymphoma by flow cytometry. To date, these studies were only performed in one laboratory at the United States of America (University of Washington). The results we obtained were recently presented in flow cytometry meetings and a manuscript is now in preparation.

From 1999 till 2006, she integrated the EWGCCA, where she represented Portugal in the Leukemia Lymphoma Immunophenotyping Task Force. EWGCCA was formed in 1996 with a Biomed-2 grant to establish collaboration between sixteen European centers that used and developed flow cytometric applications mainly for clinical purposes. The successful EC-funded multicenter Eurostandards project, concluded in 2004, was one of EWGCCA’s major achievements. The EWGCCA was extinguished in 2006, when it was established as a scientific society – ESCCA, European Society for Clinical Cell Analysis, which continued and expanded the original scope of EWGCCA (https://escca.eu/about).

European Competence Network on Mastocytosis (ECNM)

Since 2004, she integrates the ECNM, as responsible for a Portuguese Reference Center for patients with Mastocytosis (http://ecnm.net/homepage/index.php/centers-of-excellence). The ECNM, created in 2002, is a ´non-profit´ cooperative initiative of a group of scientists and clinicians in Europe who are devoted to merge their efforts to improve recognition, diagnosis and therapy in patients with mastocytosis. Specific aims in the ECNM are to provide all relevant Information to provide access to important diagnostic tests, to establish standards for the diagnosis and treatment of mastocytosis, to establish Reference Centers and Centers of Excellence in Europe, and to facilitate referrals to specialists in these centers for all patients.

Euroflow Consortium

Since 2009, she integrates, the Euroflow consortium, where she represents Portugal in lymphoproliferative disorders of T- and NK-cells (https://euroflow.org/usr/pub/participants.php). The EuroFlow Consortium was created in 2006 (EU-FP6, LSHB-CT-2006-018708) and is now a division of ESLHO (European Scientific foundation for Laboratory HematoOncology). It consists of 20 diagnostic research groups and one associated SME, which are regarded as experts in the fields of flow cytometric and molecular diagnostics. The main objective is the development and standardization of fast, accurate, and highly sensitive flow cytometric tests for diagnosis and prognostic evaluation of hematological malignancies as well as for evaluation of treatment effectiveness during follow-up.

Since 2016, she integrates the EuroBloodNet as responsible for a Portuguese Reference Center in Mastocytosis and Cutaneous Lymphoma (https://www.eurobloodnet.eu/members). The EuroBloodNet, the ERN in Rare Hematological Diseases was officially approved by the European Commission on December 2016 and started its activity 1st March 2017. It encompasses oncological and non-oncological rare hematological diseases including rare anemias, rare coagulation disorders, polycythemia, and myeloid and lymphoid tumors, and other rare hematological diseases (RHD).
